臨床研究を行う意義の一つは、私たちが診療や教育を通じて積み上げてきた家庭医療の発信にあると考えています。ですから私たちが取り上げるテーマもまた、何か新しい手技や診断方法の発見よりも、私たちが日々提供している診療をテーマにすることが多く、データを集めたり、インタビューを通して家庭医療の専門性や役割を明らかにし、アカデミックに整理することを試みています。

たとえば私は「家庭医療診療所に10年以上かかっている患者さんは、家庭医療をどのように認識しているのか？」をテーマに研究を行いました。

私が院長を務める本輪西ファミリークリニックは、人口約８万人の室蘭市にある本輪西地区で30年近く家庭医療を提供していますが、果たして当院に10年以上かかっている患者さんは家庭医療をどのように認識しているのだろう？と疑問に思ったのがきっかけです。

質問紙とインタビューによる調査の結果、患者さんは家庭医療の特徴について次の2つのとらえ方をしていることが分かりました。一つは「全部診てくれて場合によっては紹介」、あと一つは「家庭の医療」です。
前者は家庭医療の包括性や協調性と親和性のある表現であり、私たち家庭医が考える概念とも一致していました。ユニークなのは後者。患者さんは家庭医の専門性を「家庭の医療」、つまり自分たちの足腰が弱り、通院できなくなったときに「家に来てくれる（訪問診療）」のが家庭医なんだと認識していたわけです。

実際、当院では強化型在宅療養支援診療所として訪問診療を行っていますが、患者さんは私たち家庭医に最期まで「ずっと診てくれる安心感」を求めていることに気づかせてもらいました。ここから、総合診療・家庭医療を地域で広めていく上で、「家庭の医療」という切り口は発信の仕方として有用かもしれないという仮説を立てることもできるわけです。

普段私たちが行っていることを研究という形で世の中に発表していく。家庭医療の専門性を学術的に明らかにして発信していく。そうした積み重ねが、19番目の基本領域である家庭医療／総合診療の学術的な位置付けを確かなものにすることにつながり、あるいは卒前医学教育の中でもっと積極的にプライマリ・ケアが取り入れられる契機になれば、この領域を志す医学生や研修医も増えることでしょう。

研究テーマによっては、医療界だけではなく、政策や住民生活に影響を与えることがあるかもしれません。

家庭医療の診療現場で日々臨床や教育を実践しながら、さらに研究を行うのは、たとえその意義を理解していたとしても医師にとってはなかなかハードです。学術・研究支援センターでは研究しやすい環境の整備や論文執筆のサポート等を通して、HCFMに所属する医師の研究を支援し、家庭医療の普及・発展に寄与できるよう努めてまいります。

We believe that one of the reasons for conducting clinical research is to disseminate the knowledge of family medicine that we have accumulated through our practice and education. Therefore, the topics we take up are also often about the practice we provide on a daily basis rather than the discovery of some new procedure or diagnostic method, and we attempt to clarify and academically organize the specialty and role of family medicine through data collection and interviews.

For example, I asked, "How do patients who have been seeing a family medicine practice for more than 10 years perceive family medicine?" as the theme of my research.

The Motowanishi Family Clinic, of which I am the director, has been providing family medicine for nearly 30 years in the Motowanishi area of Muroran City, which has a population of about 80,000. This was the clue of the survey.

The results of the questionnaire and interviews revealed that patients have the following two perceptions of the characteristics of family medicine. One is "seeing the whole person and referring suitably " and the other is "medical care at home".

The former was an expression that had an affinity with the comprehensiveness and collaboration of family medicine and was consistent with what we family physicians consider the concept. What is unique is the latter. Patients perceived the specialty of family physicians as "medical care at home," i.e., family physicians who "come to your house (home visits)" when they themselves become too weak to go to the clinic.

In fact, our clinic provides home visits as a strengthened home care support clinic, and we have realized that patients are looking to us, family doctors, for "a sense of security that they will be taken care of all the way to the end of their lives. From this, we can hypothesize that the "medical care at home" approach may be a useful way of communicating the spread of family medicine in the community.

We will present what we usually do to the world in the form of research. We will academically clarify the specialty of family medicine and disseminate it. If these efforts lead to the academic positioning of family medicine/general practice as the 19th basic field, or if they provide an opportunity for primary care to be more actively incorporated into pre-graduate medical education, the number of medical students and residents who aspire to this field will increase.

Some research topics may have an impact not only on the medical community, but also on policy and the lives of residents.

It is quite hard for physicians to conduct research while practicing clinical practice and education at family medicine settings, even if they understand the significance of such work. The Academic and Research Centre will support the research of physicians affiliated with the HCFM by providing an environment conducive to research and support for writing papers, and will strive to contribute to the spread and development of family medicine.
